Scheduling an Appointment
You may request an appointment by phone or online. By calling our office, at (615) 865-1732, you will have the chance to speak directly with our scheduling coordinator, who will be happy to assist you in finding an appointment time that best suits your needs. Or through a simple process, you may request an appointment via our website.
Our office hours are Monday through Thursday from 8:00 a.m. to 4:00 p.m. We are also open every other Friday, 8:00 a.m. to 4:00 p.m. We close the office for lunch from 12:00 p.m. to 1:00 p.m.
We ask that you to understand a scheduled appointment is time we have reserved for you. If you cannot make an appointment, we respectfully ask to be given 24 hours notice. A broken appointment fee may be applied to your account for an appointment missed without adequate notice.
For your convenience, below is the new patient paperwork so that you may print and complete the forms prior to your first appointment. The form called 'Practice Information' provides more details about our office and policies. Additionally, patients with dental insurance will need to bring their dental card to each appointment. We ask that new patients arrive a few minutes early so that we can welcome you to our practice and process your paperwork.

Quality Dental Care at a Reasonable Cost
We strive to deliver quality dental care at a reasonable cost to our patients.
For your convenience, we gladly accept Cash, Checks, Visa, MasterCard, Discover, American Express, CareCredit and Lending Club. Payment is due at the time services are rendered unless other arrangements have been made in advance. We also accept most traditional insurance plans. Please contact our office to verify acceptance of your plan.
If you have questions about your account or balance, whether it is related to charges, insurance benefits or payments, please call us directly at 615-865-1732. Often, a simple telephone call will help to answer questions and clear up any concerns. We desire to have good communication with each patient, and are happy to be available to help in any way needed.
We are pleased to offer our patients two financing companies that will allow 0% financing for your dental treatment. Each company offers different terms and options. We would be happy to discuss the benifits of each credit company based on your personal treatment plan and needs. With either option, you can have the dental care that you need now, for a monthly payment that easily fits into your budget.

Lending Club is a finance company that allows longer payment plans to lower your monthly payment. Lending Club offers 6, 12, 18, and 24 months of interest free financing. Dental insurance is intended to cover some of the cost of your dental care. Most plans will have a deductible and co-pay that is due from the patient. As a courtesy to our patients, we will contact your insurance company and acquire a breakdown of dental benefits. Our goal is to be able to inform you of your benefits and how your dental plan works. We can generally provide an estimate of coverage and patient portion for each visit. We take pride in the care of our patients and will do our best to make sure your benefits are maximized and claims are handled correctly. Please understand that ultimately, you are the responsible party for your account with our office. You will be asked to pay your estimated portion of services the day they are rendered.
